Hustle in Pompano LLC Photo Release Form

 

I grant Hustle in Pompano the right to take, edit, alter, copy, exhibit, publish, distribute and make use of any and all pictures and video taken during the Hustle in Pompano Events. Such distribution may occur but not limited to flyers and social media without payment or any other consideration. The authorization will continue indefinitely. I hold Hustle in Pompano harmless and release from all liability.

Hustle in Pompano LLC Liability Waiver

 

I acknowledge the contagious nature of COVID-19 and its variant strains and RSV, and voluntarily assume the risk that I may be exposed to or infected by COVID-19 or RSV while participating in Hustle in Pompano and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19, RSV, and its variant strains at Hustle in Pompano may result from the actions, omissions, or negligence of myself and others, including, but not limited to Hustle in Pompano and its respective employees, volunteers, and program participants and their families.

I voluntarily agree to assume all the foregoing risks of Covid and other communicable diseases or injury from dancing and accept sole responsibility for any injury to me (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I may experience or incur in connection with my attendance at any Hustle in Pompano events.

Further, as a condition of my attendance and participation in this meeting or event, I hereby forever release, covenant not to sue, discharge, and waive any claim I may have against Hustle in Pompano and its officers, directors, employees, volunteers, sponsors, exhibitors, or agents, or instructors and forever release and hold harmless these Released Parties from and against any and all liability for damage, illness, or injury that may directly or indirectly arise out of or relate to my participation or attendance at Hustle into Pompano.

I acknowledge that participation in Hustle in Pompano events and activities necessarily involves risk and I do hereby assume full responsibility for my own well-being.

I forever release, covenant not to sue, discharge and waive any claims against Hustle in Pompano LLC, Mary Ann Rosa, or anybody associated with the Hustle in Pompano event.

This waiver and release are binding on me and my heirs and successors.
